Case 3 - COVID-19 Pneumonia

EXAM: XR Chest 1 View Portable
CLINICAL INDICATION: Shortness of breath, nausea or vomiting, body aches and
Covid positive
FINDINGS:
Support Devices: None
Lungs/pleura: No pneumothorax. There are predominantly peripheral patchy
opacities bilaterally.
Heart/mediastinum: Normal.
Other: None
IMPRESSION:
Bilateral patchy predominantly peripheral opacities.
These imaging findings are nonspecific but suggestive of atypical/viral
pneumonia (including COVID-19) in the correct clinical context. Consider
diagnostic testing and droplet precautions as appropriate.

EXAM: CT Chest w/ Contrast
CLINICAL INDICATION: PE Protocol;Acute shortness of breath.
TECHNIQUE: Multiple-row detector helical CT examination of the chest with IV
contrast. Axial, sagittal, and coronal reconstructed images. If applicable,
point-of-care testing was approved following departmental protocol.
COMPARISON: None.
FINDINGS:
Vascular: Evaluation of the pulmonary arteries is diagnostic to the subsegmental
arterial level. No pulmonary embolism. The main pulmonary artery is normal in
caliber. The aorta is normal in course and caliber.
Mediastinum: Normal heart size. No pericardial effusion. There is a moderate
size hiatal hernia.
Lymph nodes: No pathologically enlarged thoracic lymph nodes.
Airways: Central airways are patent.
Lungs/Pleura: Bilateral peripherally predominant ground glass and consolidative
pulmonary opacities. These imaging findings are nonspecific but suggestive of
atypical/viral pneumonia (including COVID-19) in the correct clinical context.
No pulmonary edema. No pleural effusion. No pneumothorax.
Upper abdomen: There is a 1.6 cm simple cyst in the left hepatic lobe. The
remaining imaged upper abdominal organs are unremarkable.
Bones and soft tissues: Imaged thyroid gland is normal. No acute osseous
abnormality. Moderate degenerative changes are present in the thoracic spine.
Prominent nonenlarged bilateral axillary lymph nodes are nonspecific but most
likely reactive.
IMPRESSION:
1. No pulmonary embolism.
2. Commonly reported imaging features of COVID-19 pneumonia or other viral
pneumonia.
3. Moderate size hiatal hernia.
